Many butt plugs have a recess in the bottom in which you can insert a standard bullet vibe.  These come in a great variety of shapes and sizes... for your comfort and pleasure ; )

There is no need to have a small, close fitting condom.  Just make sure it encases the base of the toy.  Though if you are still concerned... yes they make small condoms...

If you really want something *soft*, there are lots of options. Blowfish is a great site and I should really get kickbacks from them... Anything smallish with a nicely sized flared base will work great. Most of the time I find that things that have the special "prostrate gland" shape to them tend to spin around inside during use and then you can't figure out which way they are supposed to go anyways... so flexibility I think is more important than special prostrate designs.

Edited to add... re: condoms, you should always use them on your toys... makes them so much easier to clean! The vast majority of sex toys cannot be boiled or 100% sterilized. Only silicone toys can do that... so condoms are wonderful for helping to keep things clean. They don't (yet) make special condoms for toys, I bet that'll come in a few years... but Lifestyles makes a snugger fit condom, or I buy from ripnroll.com the contempo exoticas which work quite well.
