Once you enter the TER ID number the form should auto populate so you can then just write the narrative

Yeah, it's a pain in the ass filling in all that shit, but I think admin does it that way to discourage fake reviews. They don't want to make it easy to do them, so everyone has to pay the price.

The eliminates most of the shill or fake review risks, while providing a benefit to those who have paid for value.
I only filled out what I need to then I skip the rest. Just look for the "*" and skipped those out and fill in the rest. If filling out the entire profile would update the profile with current info then I would but it does nothing so I skip it.
