Living on Long Island, my "city" is New York. Now Long Island is bigger than some states with 2 distinct counties.  People travel to and from "The City" all the time, but I prefer to do most of my hobbying out in Suffolk.  The stings that happened in Nassau last year spooked a lot of providers and mongers.  With reason!  I still dislike heading into Manhattan because of the 2-3 hours each way in the car, not to mention all the things that can happen along the way.  (Car crash, flat tire, traffic preventing me from getting home before SO, and yes, terrorist attacks - I lived thru 9/11, thank God I wasn't hobbling THAT day!)
Geographically finding a provider near me takes up a lot of time due to 5 separate area codes all in the same geographical island.  Well, 2 really 516 and 631, but cell codes such as 917, 347 and 646 are ubiquitous as well.  You can put in "Long Island" in the search but that excludes providers who day trip from the City.

Wish it was easier.  Though I get the idea of the poll, I look for proximity, daytime availability, then reviews/score, then price

Because my "City" is actually a whole geographic region "New England" it is NECCESSARY top scan the area codes & locations on reviews for a hint that a provider is actually near me!   This glitch skews the results.
After I determine if she's near me, I look at other attributes.

Maybe one value to the poll is getting a pie chart on how many have to deal with this because the areas are to large, or rather that they can't be narrowed down. Maybe All Burroughs could be one option along with Manhattan, Brooklyn, The Bronx, whatever.
