I have a handful of inactives too.  That is probably normal.  Where I could get better is having provider update my okay after most recent outing.  

Everyone is different in review criteria.  That is okay.  All have to be comfortable.

all ladies are different. inactive oks are super common.  
i can see someone being concerned about your "5" in your review, but if someone clicked on that ladies reviews, they would see that you were just part of the norm for her.

I have many.  In fact,  most. All of my ATFs either went to Vegas or California, or just disappeared. That is normal and will happen to anyone who has had an account for a while.  

Depending on how much you date, I suggest getting a new OK every 3 months.  Don't go over 6 months ever. The longer you go between dates and having an OK,  the more likely the top providers are to distrust you,  or your last dates my not remember you. Then you may have to go to newbie friendly girls and miss out on top rated girls you may want to see.  

You can get a updated provider OK from a regular too. Always send your requests and keep the references recent. Also,  many independent girls don't trust a reference from an agency. Just know that if you do use agencies a lot, you can have a harder time with screening with a lack of preferred,  or trustworthy references for top rated girls.
