Those are my top 4. The rest don't matter much. I much prefer independents to agency.

Based on the variables you posted, here is their importance to me, from most to least, in my decision making process:
1. Pictures (hopefully accurate) -no attraction, I move on.
2. Location - not so much geographical, but must be incall. If outcall only, I'll move on.
3. Service menu - as others have said BBBJ and DFK are essential.
4. Reviews - I always read the lowest scores first.
5. Price
6. Independent vs Agency vs Assistant - I've used all three, and each have their own set of pros and cons. Overall, I probably prefer independent, but this is a rather low variable in my analysis.
7. Ease of scheduling and screening - For me screening isn't an issue. With some independent ladies, scheduling can be a daunting task, which is the major advantage of agencies and assistants. But, unless the effort to schedule becomes tortuous, it's not usually a big issue.

I depend on reviews as the primary method of choice. I read most of the new reviews in the evening to wind down the day. From those a provider might catch my interest for a variety of reasons and if they do I will put them into favorites for later. When I am ready I cull through the ad board here and then move to my favorites for more research. Certainly services, price and appearance factor in heavily but most often I spend time on the providers website and that can give me a sense of the Ladies personality and whether they are someone I could feel comfortable with. Taking your site as an example there are several ladies represented there; all very attractive but some just have pictures and basic stats. Hard to get any sense of attraction there. The ones I would see have at least some type of description or personal note about themselves. No description, no reviews equals no business.

Looks are important - that's part of why I do this. So is service - I want the stuff I can't get at home. Rate is important - too cheap and it's scary but too expensive and I can't hobby as often. Location isn't huge - I can travel or have outcall. Lastly I need some kind of review so I know the provider is what I am looking for. No bait n switch, no promise of bbbj but get "I don't do that anymore" type thing.

+1 for me on SKOC's reply. I go through pretty much the same criteria. Never used an agency yet though. Still fairly new and have had good luck with the independents that I've contacted.
In order from first considered to last:
Pictures - attraction is a must, but I have quirky tastes sometimes
Reviews - one or two recent good (6+) reviews is a must
Independent/Agency/Assistant - Typically prefer either independent or assistant
Services - I look for a few specific services, depending on my mood
Location - Also depends on my mood/ free time etc.
Price - should be in the current hourly going-price rang
